چکیده

Introduction Balanced solutions may be associated with lower morbidity in critical illness in comparison with chloride-rich solutions, including acute kidney injury (AKI). Lactated ringer (LR) is one of most widely used balanced solutions. Nevertheless, it is unclear if fluid mix during the first days of ICU admission influences the development of AKI after controlling for other relevant confounders such as fluid balance, illness severity and comorbidities.

Risk Factors of Acute Kidney Injury in Patients with Sepsis; A Cross-Sectional Study

Background and Objective: Due to the importance of acute kidney injury associated in patients with sepsis and the impact of various factors on mortality and hospital stay of these patients, this study was conducted to investigate the clinical features and risk factors for acute kidney injury in patients with sepsis. In addition, we examined whether the severity of acute kidney injury affected c...

ارتباط بین غلظت هموگلوبین و مورتالیتی در بیماران ترومای مغزی بستری در بخش مراقبت‌های ویژه

Background and Objective: Traumatic brain injury is one of the main causes of mortality and morbidity worldwide and the second leading cause of death in Iran. About half of patients with traumatic brain injury have hemoglobin of less than 9 g/dL during the first week of admission. With regard to the secondary damage to brain tissues caused by anemia and blood transfusion complications, we decid...

Abnormal saline: the unphysiological bag of brine

Duburcq and colleagues, in their article published in Critical Care, showed improved oxygenation, hemodynamic parameters, and microvascular reactivity in an experimental porcine endotoxic shock model with use of hypertonic sodium lactate 11.2% [1]. We applaud the authors for embarking on the arduous quest for the ideal resuscitative fluid, which has historically been fraught with shortcomings. ...

Balanced Crystalloid Use Is Associated With Haemodynamic Stability And Less Need For Vasopressors in Patients Receiving Renal Transplantation Compared To 0.9% Saline.

Background Data is suggesting that non-balanced conventional crystalloids (0.9% saline) are not only associated with the occurrence of hyperchloremic, metabolic acidosis but increased incidence of acute kidney injury and even morbidity and mortality. Experimental data suggested infusion of hyperchloremic crystalloids to be associated with hypotension in experimental sepsis. We aimed to investig...

Positive fluid balance as a prognostic factor for mortality and acute kidney injury in severe sepsis and septic shock.

PURPOSE The purpose of this study is to assess whether late positive fluid balances are associated with acute kidney injury and mortality in severe sepsis and septic shock. METHODS In this retrospective study, fluid balances were calculated at 3 different time points: the onset of organ dysfunction attributed to sepsis, sepsis diagnosis, and vasopressors initiation. Data were analyzed in logi...
